Ultrasonography (US) has long been considered an important diagnostic imaging modality for investigation of the pancreas despite certain significant and well-known limitations. Indeed, in many countries US represents the first step in the diagnostic algorithm for pancreatic pathologies. Recent years have witnessed major advances in conventional, harmonic, and Doppler imaging.

New technologies, softwares, and techniques, such as volumetric imaging, enhancement quantification, and fusion imaging, are increasing the diagnostic capabilities of US. The injection of microbubble contrast agents allows better tissue characterization with definitive differentiation between solid and cystic lesions. Contrast-enhanced US improves the characterization of pancreatic tumors, assists in local and liver staging, and can offer savings in both time and money. Acoustic radiation force impulse (ARFI) imaging is a promising new US method to test, without manual compression, the mechanical strain properties of deep tissues. Furthermore, the applications and indications for interventional, endoscopic, and intraoperative US have undergone significant improvement and refinement.

About the Author
  • G. B. Rossi University Hospital is a referral center specialized in pancreatic diseases. In 2006, the Department of Radiology performed more than 900 contrast-enhanced ultrasound examinations, half of which were for pancreatic pathologies, and 200 ultrasound-guided biopsies for pancreatic masses. 
  • Dr D’Onofrio has renowned international experience in pancreatic disease management and in the use of contrast-enhanced ultrasound. All ultrasound studies of pancreatic lesions need to be correlated with the findings of other imaging modalities in order to reach a final diagnosis. A complete description of the correlations between ultrasound-imaging and ultrasound-pathologic findings will lead to an improvement of the diagnostic capability of ultrasound in studying pancreatic pathologies. Dr D’Onofrio has been working since 2004 as a university researcher in the Department of Radiology at the University of Verona in the University Hospital of G. B. Rossi. He has been an associate of SIRM (Italian Society of Medical Radiology) and AISP (Italian Association for the Study of the Pancreas) since 2002 and is a member of the editorial board of the official journal of SIRM. He has been a faculty member of IBISCUS (International Bracco Imaging School for Contrast-Enhanced Ultrasound) since 2003. He is an associate of SIUMB (Italian Society of Ultrasound in Medicine and Biology) and EFSUMB (European Federation of Societies for Ultrasound in Medicine and Biology) and a member of ESGAR (European Society of Gastrointestinal and Abdominal Imaging). He is the chairman for pancreatic applications of contrast-enhanced ultrasound in the EFSUMB guidelines and for the good clinical practice recommendation in contrast-enhanced ultrasound meetings update 2007. Dr D’Onofrio is author of more than 40 original papers in Italian and international medical journals and of 7 book chapters; he is also the author of the book Ecografia e procedure interventistiche percutanee, published by Springer in 2008. In addition, he is a reviewer for AJR. American Journal of Roentgenology, Pancreas, CMAJ: Canadian Medical Association Journal, BMC Cancer, and La Radiologia Medica.

Practical Ultrasound in Anesthesia for Critical Care and Pain Management

Endoscopic ultrasound (EUS) was first conceptualized almost 30 years ago during the early years of endoscopy and was developed in an attempt to improve ultrasound imaging of the pancreas. The first EUS prototype was designed and manufactured in the early 1980s. Since that first prototype, endoscopic ultrasoundhas evolved into an accepted and valuable endoscopic modality and subsequently, a therapeutic modality for the management of many gastrointestinal disorders. EUS is now entering a new era in which the procedure is leaving the confines of specialized tertiary referral centers and academic institutions, and becoming disseminated in community based hospitals throughout the world.

Additionally, the technology is now progressing from that of diagnosis and staging, to a more interventional therapeutic role. Ultrasound Diagnostics of Thyroid Diseases

Thyroid disease is the second most common type of endocrine pathology, only surpassed in prevalence by diabetes mellitus. Thyroid abnormalities are found in 8–20% of adults worldwide. In the endemic regions, the prevalence of such abnormalities is thought to be higher and exceeds 50%. Thyroid malignancies constitute 1–3% of all cancers with an average incidence in the world of 1.1 in 100,000 men and 3.8 in 100,000 women in 2008. Among the population of radionuclide polluted regions, this figure reaches 14 in 100,000. Recent research reveals a trend toward an increased incidence of thyroid pathology, including thyroid cancer, practically in all regions of the globe.
The diagnosis of thyroid diseases has been constantly improving due to the scientific development and technological advances in diagnostic equipment. The diagnostic value of visualization of the thyroid gland is method-dependent. In this regard, proper selection of a diagnostic procedure permits precise diagnosis while minimizing the cost and reducing the time to diagnosis. Minimally invasive surgical intervention is a promising tool in the treatment of thyroid diseases. Its feature is selective manipulation of the thyroid lesions and concomitant avoidance of damage to the surrounding tissue. The use of US guidance during such procedure allows to assess the operation course, predict the efficacy, and provide patient follow-up. This book we present and analyze certain debatable and unresolved problems and prospects of early, specific, and differential diagnosis of thyroid disease with the use of complex US.

High-resolution Ultrasonography for Peripheral Nerve Diagnostics: A Guide for Clinicians Involved in Diagnosis and Management of Peripheral Nerve Disorders

The clinical neurophysiologist is primarily concerned with being able to provide an accurate diagnosis. Defining the location and extent of a lesion is achieved through a detailed clinical examination with judicious application of additional tests of nervous function. The role of nerve conduction studies and the related techniques of EMG evoked potentials in providing further functional information has been well established. Recently, however, technical advances in imaging techniques have enhanced the scope for providing the clinician with much-improved anatomical information regarding disorders of the peripheral nervous system. The technical advances of high resolution ultrasonography (HRU) have been nothing less than spectacular, enabling hugely improved depiction of peripheral nervous structures in a three-dimensional setting. This technique is set to progress rapidly and become an established adjunct of the clinical neurophysiologist. Combination with color Doppler allows integration of additional information on the vascular supply of the peripheral nerves.
However, the main advance brought about by HRU is that it enables accurate localization of peripheral nerve disorders. In addition, it is cheap and readily available, in contradistinction to the costly MRI techniques. A further advantage is that it allows dynamic, real time injections of nerves, which are frequently performed by anesthetists.

Endoscopic Ultrasonography 2nd edition

Endoscopic ultrasound (EUS) was first conceptualized almost 30 years ago during the early years of endoscopy and was developed in an attempt to improve ultrasound imaging of the pancreas. The fi st EUS prototype was designed and manufactured in the early 1980s. Since that first prototype, endoscopic ultrasound has evolved into an accepted and valuable endoscopic modality and subsequently, a therapeutic modality for the management of many gastrointestinal disorders.
